Dr. Cook received his undergraduate degree from Wabash College, doctor of medicine from Indiana University, and master of public health degree from the Medical College of Wisconsin. He completed his internal medicine training at the Methodist Hospital of Indiana and his pulmonary /critical care fellowship at the University of California (Davis/Sacramento Medical Center).
He has served as Chief Medical Officer for The Key Companies since 1997. There he has worked with information technology professionals and clinical staff to develop and evolve analytic reporting and consulting services for self-funded employers, disease and demand management programs, and provider practice analytics.
He was a founding member of ICareConnect, Inc., a group led by physicians to organize a community health information exchange. ICareConnect was one of three entities that merged into Indiana Health Information Exchange (IHIE). David is now on the Board of Directors of IHIE with a special interest in quality/value performance, evidence-based medicine, and physician engagement. Currently, he uses his unique expertise to assist health information exchanges and their participating organizations to assess the financial benefits of participation in a health information exchange.

officer with the United States Public Health service detailed
to the United States Coast Guard as a Flight Surgeon and Diving
Medical Officer at the San Francisco Air Search and Rescue station
and in Operation Deep Freeze ’74 in Antarctica
- Medical Director of Respiratory Care at Methodist Hospital
of Indiana from 1979 to 1994
- Founding member and managing partner of the Respiratory
& Critical Care Consultants group practice
- Director of Quality Management at St. Vincent Hospital
from 1994 to 1997
- Past president of the Indiana Thoracic Society
- Member of the Boards of Directors of the:

Lung Association of Indiana from 1981 to 1989
National Association of Medical Directors of Respiratory Care
